Asthenopia

Why it occurs
- Uncorrected or inadequately corrected refractive error (latent hyperopia, astigmatism, incipient presbyopia)
- Insufficient ocular convergence or decompensated phorias due to fixation effort
- Computer vision syndrome (digital eye strain caused by significantly reduced spontaneous blinking when fixating on electronic screens)
- Mild evaporative keratoconjunctivitis sicca
- Unfavorable ambient lighting conditions or presence of annoying reflections in the field of vision
Initial workup
Complete computerized and subjective refractive examination under pharmacological cycloplegia (to unmask latent hyperopia, especially in children and young adults); Quantitative orthoptic evaluation (measuring extrinsic ocular motility, the proximal point of convergence - PPC, and the presence of latent deviations through cover testing); Qualitative evaluation of the tear film (TBUT, Schirmer).
red flags
Progressive eye fatigue and persistent frontal headache that is accompanied by transient or constant binocular diplopia when performing near tasks, or occasional oscillopsia when fixating gaze, suggesting subtle paresis of oculomotor nerves or myasthenia gravis.
Standard management
- Sodium hyaluronate — 0.18% or 0.20% eye drops without preservatives, apply 1 drop every 2-4 hours preventively during periods of reading or sustained use of screens to restore the evaporated tear film
- Artificial tears with 0.5% carboxymethylcellulose (apply according to clinical demand). Primary management includes non-pharmacological ergonomic measures such as the 20-20-20 rule (resting your eyes every 20 minutes by looking 20 feet/6 meters for 20 seconds) and accurate optical correction in reading glasses or screens.
