Extreme evening fatigue

Why it occurs
- Relative adrenal dysfunction or flattened rhythm of cortisol secretion (sudden and early drop in cortisol levels in the afternoon instead of a gradual physiological decrease)
- Attention deficit hyperactivity disorder (ADHD) in adults (exhaustion of compensatory mental effort accumulated throughout the work day)
- Hidden non-restorative sleep disorders (sleep microapneas that manifest as extreme fatigue accumulated in the afternoon after daily activities)
- Late postprandial reactive hypoglycemia (sudden drop in serum glucose 3 to 4 hours after lunch due to reactive hyperinsulinism)
- Consumption of psychotropic or antihypertensive drugs with evening plasma peaks that cause delayed sedation.
Initial workup
Salivary cortisol curve at four points (morning, noon, afternoon, night), continuous monitoring of capillary blood glucose or 5-hour oral glucose tolerance test, Epworth daytime sleepiness scale, and overnight polysomnography.
red flags
Presence of severe mental confusion in the afternoon ("disabling brain fog"), marked postural arterial hypotension with presyncope when rising from the work chair, mild exertional dyspnea, or associated rapid palpitations.
Standard management
- Avoid short-acting stimulants (such as caffeine in high doses in the afternoon) that worsen the inertia of subsequent nocturnal sleep
- Adjust the administration schedule of antihypertensives or sedatives from the afternoon to the evening under medical indication
- B complex supplementation (formulas with B1, B6 and B12 in the morning and noon to support energy metabolism).
