  • Cranial nerve paresis (III, IV, VI)
  • Myasthenia gravis
  • Thyroid orbitopathy
  • Trunk injury

Initial workup

Cover each eye (mono/binocular), explore cranial nerves; neuroimaging if binocular.

red flags

Sudden diplopia with headache, ptosis or mydriasis (aneurysm/III nerve): urgency.

Standard management

  • Urgent study of the cause — there is no symptomatic drug
