Nodal or bone pain induced by alcohol intake

Why it occurs
- Hodgkin's lymphoma (classic and highly specific symptom, although rare; alcohol induces local vasodilation in lymph nodes infiltrated by Reed-Sternberg cells, tensioning the lymph node capsule that is already under pressure due to the reactive inflammatory infiltrate)
- Non-Hodgkin lymphoma (less common, occasionally seen in large or follicular B-cell variants)
- Boeck's sarcoid / Sarcoidosis (systemic granulomatous inflammatory reaction that simulates lymph node neoplastic behavior)
- Chronic active osteomyelitis
- Langerhans cell histiocytosis.
Initial workup
Excisional biopsy of the complete lymph node (never fine needle puncture-aspiration, to preserve the tissue architecture essential for the anatomopathological and immunohistochemical diagnosis that demonstrates the presence of CD15+ and CD30+ Reed-Sternberg cells); computed tomography (CT) of the neck, thorax, abdomen and pelvis with intravenous contrast for tumor staging; positron emission tomography combined with CT (PET-CT) with fluorodeoxyglucose (FDG) to delineate the metabolic extent of nodal and extranodal disease; complete blood count, erythrocyte sedimentation rate (elevated ESR, prognostic marker in Hodgkin Lymphoma) and lactate dehydrogenase (LDH, indicator of cell turnover and tumor burden) levels.
red flags
Presence of severe substernal chest pain or restrictive dyspnea immediately after alcohol ingestion in a patient with known mediastinal masses (due to acute expansion of the intrathoracic lymph node volume with compression of the superior vena cava or the main airway); coexistence of classic B symptoms of neoplastic origin (intermittent fever predominantly in the afternoon, profuse night sweats that require changing bed linen, and involuntary weight loss of more than 10% of body mass in the last six months); appearance of severe cytopenias in the blood count suggesting diffuse tumor infiltration of the bone marrow.
Standard management
- ABVD Scheme — systemic combined chemotherapy composed of Doxorubicin, Bleomycin, Vinblastine and Dacarbazine; Standardized first-line therapeutic regimen for classical Hodgkin lymphoma
- Brentuximab vedotin — conjugated antibody directed against CD30, used in advanced or refractory phases associated with chemotherapy
- Prednisone — systemic glucocorticoid that rapidly decreases the periganglionic reactive inflammatory component, reducing the painful sensitivity of the lymph node capsule
- Paracetamol / Acetaminophen (non-anti-inflammatory analgesic for the palliative symptomatic management of alcohol-induced pain while definitive oncological treatment is initiated).
