Why it occurs

  • Autism spectrum disorder (ASD)
  • Catatonic schizophrenia
  • Advanced dementia (Alzheimer or vascular type)
  • Gilles de the Tourette syndrome
  • Transcortical type aphasia (sensory or mixed)
  • Metabolic or toxic encephalopathies

Initial workup

Neuropsychological evaluation of language; Brain MRI to evaluate cortical lesions of the dominant hemisphere; electroencephalogram; Bush-Francis catatonia scale.

red flags

Association with echopraxia (imitation of movements) and catatonic muscle rigidity, alternating mutism or uncontrollable psychomotor agitation.

Standard management

  • Treat the underlying pathology. If it is framed in a catatonic condition: Lorazepam — benzodiazepine at medium-high doses
  • If associated with neurodevelopmental disorders with agitation or impulsivity: Risperidone or Aripiprazole at low doses.
