Why it occurs
- Manic or mixed episode of bipolar disorder
- Acute intoxication by stimulants (amphetamines, cocaine, methylphenidate, ecstasy)
- Serotonin syndrome (initial phase)
- Delirium of organic or infectious origin
- Schizoaffective disorder
Initial workup
Vital signs (rule out tachycardia, fever, hypertension associated with intoxication or serotonin syndrome); YMRS scale (mania); analysis with urine toxicants and serum lithium levels (if the patient was already on treatment).
red flags
Absolute incoherence of speech that prevents any type of communication, inability to concentrate attention on risks in the environment (crossing streets, operating machinery), or dehydration and physical exhaustion due to incessant tachypsychia.
Standard management
- Olanzapine — atypical antipsychotic with powerful sedative and thought-regulating effect
- Haloperidol intramuscularly or intravenously — if there is unmanageable psychomotor agitation
- Sodium Valproate or Lithium Carbonate — to start or adjust mood stabilization
- Lorazepam — to attenuate cerebral hyperactivity and promote rest
