Galactorrhea

Why it occurs

  • Hyperprolactinemia (prolactinoma)
  • Drugs (antipsychotics, metoclopramide)
  • Hypothyroidism
  • Breast stimulation

Initial workup

Pregnancy, prolactin and TSH test; review medications; Pituitary MRI if prolactin is very high.

red flags

Galactorrhea with amenorrhea and visual field alteration (macroprolactinoma).

Standard management

  • Dopamine agonist — cabergoline in prolactinoma
  • Review/remove offending drugs
