Why it occurs
- Physiological variation of connective tissue (more elastic collagen)
- Benign joint hypermobility syndrome
- Hereditary connective tissue disorders (Ehlers-Danlos hypermobility syndrome, Marfan syndrome, Osteogenesis imperfecta)
- Specific physical training (gymnasts, classical ballet dancers)
- Constitutional muscle hypotonia or central neurological origin
Initial workup
Quantitative clinical evaluation using the Beighton Scale (score greater than or equal to 5/9 in adults indicates generalized hypermobility) | Transthoracic echocardiogram (indicated to rule out aortic root dilation or mitral valve prolapse) | Genetic panel for connective tissue diseases in cases with suspected syndromic inheritance.
red flags
Extreme joint laxity associated with sudden tearing chest pain or dyspnea (suspected aortic dissection in a patient with suspected Marfan or vascular Ehlers-Danlos), or daily multiple and spontaneous joint dislocations with peripheral neurological damage due to traction.
Standard management
- Paracetamol — 1 g every 8 hours orally intermittently for persistent joint pain due to overuse
- Celecoxib — 100 mg every 12 hours orally in case of synovitis or bursitis reactive to episodes of subluxation
- Supplementation with vitamin D (1000 IU/day) and calcium to promote bone mineral stability.
