Initial workup

Distinguish vertigo/presyncope/imbalance; Dix-Hallpike maneuver; explore nystagmus and neurological.

red flags

Vertigo + headache, diplopia, dysarthria or ataxia → suspected central cause (stroke).

Standard management

  • Sulpiride/betahistine — peripheral vertigo
  • Replacement maneuvers — BPPV
