Why it occurs
- Lobar pneumonia in the phase of gray or red hepatization (the consolidated alveolar tissue acts as an excellent acoustic conductor, allowing the words whispered or whispered by the patient to be heard clearly and articulately through the stethoscope, which does not occur in the normal healthy lung)
- Partial obstructive pulmonary atelectasis with peripheral consolidation (acoustic transmission facilitated by loss of alveolar aeration)
- Large lung cavities with a thick wall and connected to a bronchus (the cavity acts as a resonance chamber for the acoustic frequencies of the whispered voice)
- Extensive pulmonary infarction in the organizing phase
- Localized pulmonary fibrosis of a scarring or post-infectious nature (replacement of alveoli with dense collagen tissue that improves sound conduction).
Initial workup
Chest x-ray (inspection of areas of consolidation with air bronchogram); High-resolution chest computed tomography (CT); Thoracic ultrasound (localization of areas of pulmonary hepatization); Sputum culture and cytology; Spirometry if the patient is clinically stable to evaluate associated restrictive patterns.
red flags
Presence of signs of respiratory sepsis, associated dyspnea at rest, nail cyanosis, marked use of the accessory muscles of the neck, or associated massive pleural effusion that causes deviation of the mediastinum on the chest x-ray.
Standard management
- Empirical antibiotic therapy adapted to the patient's environment — for example, Levofloxacin 500-750 mg orally or intravenously once a day if it is community-acquired pneumonia with risk factors
- Intravenous hydration optimized to promote the liquefaction of alveolar secretions.
