Why it occurs
- Primary impulse control disorder (pyromania)
- Antisocial personality or dissocial behavior disorder in adolescents
- Moderate-severe intellectual disability with fascination with fire
- Disorganized manic episode
- Substance abuse with loss of behavioral control
- Schizophrenia (due to imperative hallucinations or mystical delusions with fire)
Initial workup
Comprehensive forensic and psychiatric evaluation; ruling out child and adolescent behavioral disorders; assessment of intellectual quotient (IQ); toxicological screening.
red flags
High risk of causing human loss, destruction of critical infrastructure, self-harm due to severe burns, or immediate judicial arrest.
Standard management
- There is no specific approved pharmacological treatment. Impulsivity modulators are used: Topiramate — stabilizer that mitigates motor urgency
- Risperidone or Aripiprazole — antipsychotics to reduce maladaptive impulsivity and disruptive behaviors
- Lithium carbonate — if it is associated with extreme emotional instability or bipolar disorder
