Sensation of coordinated mental and physical fatigue

Why it occurs

  • Burnout syndrome or severe chronic work exhaustion (dysregulation of the sympathetic-adrenal system and central neurotransmitters due to sustained overload)
  • Prolonged adaptation disorders or complicated grief (intense psychological and physical reaction that generates a catabolic state and central asthenia)
  • Myalgic encephalomyelitis (neuroimmune syndrome that coordinately affects cognitive ability "brain fog" and physical tolerance)
  • Lyme disease in the late dissemination phase or neuroborreliosis (chronic inflammation of the nervous and immune system induced by Borrelia burgdorferi)
  • Severe metabolic deficiencies of trace elements due to malnutrition or chronic alcoholism.

Initial workup

Metabolic and renal profile, complete blood count, thyroid profile, antinuclear antibodies (ANA), serology for Borrelia burgdorferi if there is a history of tick bite, levels of vitamin B12 and folic acid, and brain MRI if a structural lesion is suspected.

red flags

Development of structured autolytic ideation, progressive temporal-spatial disorientation, involuntary weight loss greater than 10% in a few months, or intense recurrent headache predominantly at night.

Standard management

  • Sulbutiamine — 200 mg orally twice daily, a lipophilic thiamine derivative indicated for the treatment of residual neurophysical asthenia
  • Selective modulators such as Escitalopram — 10 mg orally daily if there is an underlying anxiety or depressive disorder that causes coordinated fatigue
  • Supplementation with Chelated Magnesium (250 to 500 mg per day orally for neuromuscular support and reduction of fatigue).
