Why it occurs

  • BPPV (benign positional)
  • Vestibular neuritis
  • Ménière's disease
  • Central vertigo (posterior fossa stroke)

Initial workup

Distinguish peripheral/central; Dix-Hallpike and nystagmus scan; neuroimaging if there are central signs.

red flags

Vertigo + headache, diplopia, dysarthria or ataxia → suspected central cause.

Standard management

  • Sulpiride / betahistine — vestibular sedatives, in the acute phase
  • Epley maneuver — repositioning in the BPPV
