Nitroglycerin and Sodium Nitroprusside

Mechanism
Pharmacological Class and Group
Direct vasodilators and synthetic nitric oxide donors. Nitroglycerin is a preferred venodilatory organic nitrate; Sodium nitroprusside is a balanced direct arterial and venous vasodilator (inorganic nitrovasodilator).
Mechanism of Action
Relaxation of smooth muscle in the wall of venous and arterial blood vessels is controlled by the nitric oxide (NO) and cyclic guanosine monophosphate (cGMP) pathway in the smooth muscle cell.
Molecular Mechanism of Nitric Oxide Donation
1. Nitroglycerin (venodilatory profile enzymatic activation): It is an organic nitrate that requires an enzymatic process of mitochondrial reduction mediated by the enzyme aldehyde dehydrogenase 2 (ALDH2) to release free nitric oxide (NO) in the cytoplasm of the smooth muscle cells of the venous wall.
2. Sodium Nitroprusside (Photo-sensitive spontaneous release): It is an inorganic complex composed of an iron atom coordinated with five cyanide groups and a nitrosyl group. Upon contact with plasma erythrocytes, it reacts spontaneously and non-enzymatically, releasing nitric oxide immediately into the surrounding environment, which explains its extreme rapidity of action and vasodilatory power.
3. Soluble Adenylate Cyclase (sGC) Pathway and Vasodilation: Free NO stimulates the cytoplasmic enzyme soluble guanylate cyclase (sGC), triggering the synthesis of cGMP from cellular GTP:

The activation of PKG promotes the phosphorylation of L-type calcium channels (Cav1.2), inhibiting the entry of calcium, and stimulates the reuptake of calcium to the sarcoplasmic reticulum by activation of the sarco-endoplasmic calcium pumps (SERCA). This drastically decreases free cytoplasmic calcium, inactivating MLCK and inducing massive relaxation of the vessel wall.
4. Differential Vessel-Selective Effect:
- Nitroglycerin: At low conventional doses, it acts selectively and dominantly on the venous capacitance vessels (venodilation). This decreases venous return, reduces left ventricular preload and decreases myocardial diastolic wall stress, reducing oxygen demand. At high doses, it also induces systemic arterial vasodilation.
- Sodium Nitroprusside: Induces a totally symmetrical and powerful arterial and venous vasodilation. This reduces preload and afterload in a balanced way, dropping blood pressure immediately in a dose-dependent manner.

High Resolution Pharmacokinetics
- Nitroglycerin (Sublingual / Intravenous): It presents an absolute first-pass hepatic clearance (oral bioavailability < 1% due to the action of the gastric and hepatic nitrate reductase enzyme), which prevents its conventional oral intake (requires sublingual transmucosal absorption or direct intravenous infusion). Its plasma elimination half-life is only 1 to 3 minutes, with a duration of its local biological effect of 10-20 minutes after the sublingual bolus.
- Sodium Nitroprusside: It is administered exclusively by continuous intravenous route with an infusion pump. Its onset of action is immediate (in seconds), reaching its maximum peak in 1 to 2 minutes of infusion. When the pump is stopped, the tension effect disappears completely within 3 to 5 minutes. Indicators and dose
Clinical Indications and Uses
- Acute Phase of Ischemic Heart Disease (Unstable Angina / AMI): Sublingual nitroglycerin or continuous intravenous infusion for the immediate control of anginal and ischemic pain.
- Acute Lung Edema Associated with Heart Failure: Intravenous nitroglycerin to rapidly reduce preload and relieve retrograde pulmonary congestion.
- Urgent Hypertensive Crises with Organ Damage (Dissecting aortic aneurysm / Hypertensive encephalopathy): Sodium nitroprusside by intravenous infusion controlled with a pump for the controlled decrease in blood pressure.
Dosage and Clinical Adjustment
Nitroglycerin:
- Sublingual (Acute Anginous Pain): 0.4 mg to 0.8 mg sublingual every 5 minutes, up to a maximum of three consecutive doses within 15 minutes of clinical suspicion.
- Intravenous infusion: Start with 5 µg to 10 µg/minute in continuous infusion, titrating upwards in steps of 5-10 µg/minute every 3-5 minutes according to the patient's hemodynamic and blood pressure response.
Sodium Nitroprusside:
- Hypertensive Crisis: Start with 0.3 µg/kg/minute in continuous intravenous infusion with an infusion pump. Maximum recommended maintenance dose of 10 µg/kg/minute. The infusion bottle must be strictly protected from direct light by wrapping it in opaque aluminum foil (due to its extreme photo-sensitivity, since exposure to light accelerates the degradation and release of cyanide in the bottle).

Absolute and Relative Contraindications
Contraindications of Nitroglycerin
- Basal profound arterial hypotension or shock of cardiogenic origin.
- Confirmed right ventricular infarction: Venous vasodilation drastically decreases venous return, fatally collapsing volume-dependent cardiac output in these patients.
- Concomitant use of Phosphodiesterase 5 Inhibitors (Sildenafil, Tadalafil): Massive synergism of cGMP accumulation that causes lethal refractory cardiovascular collapse.
Contraindications of Nitroprusside
- Advanced kidney or liver failure (critical risk of accumulation and toxicity from cyanide and thiocyanate metabolites).
- Compensatory arterial hypertension (coarctation of the aorta or arteriovenous shunt).
- Elevated baseline intracranial pressure.
Adverse Effects and Specific Toxicity
- Intense Pulsating Headache (Nitroglycerin 30%): Secondary to direct vasodilation of the meningeal arteries. It resolves with conventional analgesics.
- Orthostatic Hypotension and Reflex Tachycardia: Requires caution when incorporating the patient.
- Nitrate Tolerance (Tachyphylaxis): The continuous infusion of nitroglycerin over 12-24 hours depletes the cellular sulfhydryl groups necessary for its bioactivation by ALDH2, progressively attenuating its dilating effect. Requires nitrate-free periods of 10-12 hours a day in chronic treatments.

Critical Toxicity: Pathophysiology of Nitroprusside Cyanide Intoxication
Each molecule of sodium nitroprusside spontaneously releases 5 cyanide ions into the capillary plasma. Cyanide is taken up by the liver and conjugated by the mitochondrial enzyme rhodanase together with sulfur donors to form thiocyanate (100 times less toxic metabolite, eliminated through the kidneys). If high doses of nitroprusside are infused over a prolonged period, sulfur reserves are depleted, and free cyanide accumulates. It is clinically manifested by metabolic acidosis with unexplained elevated GAP anion, psychomotor agitation, bitter almond breath, mental confusion and cardiovascular collapse. Management requires immediate discontinuation of the drug and administration of hydroxocobalamin (Vitamin B12a), which selectively binds to plasma cyanide to form non-toxic cyanocobalamin, safely eliminated in urine.
Relevant Drug Interactions
- PDE5 inhibitors (Sildenafil): Sildenafil blocks the degradation of cGMP. If co-administered with an NO donor (nitroglycerin), the accumulation of cGMP is massive and uncontrollable, inducing profound hypotension and shock that is difficult to manage:

When presenting with acute chest pain in the emergency department, it is mandatory to actively question the patient about recent use of phosphodiesterase 5 inhibitors for erectile dysfunction before administering nitroglycerin. The following exclusion deadlines must be strictly complied with:
