Alendronate Sodium and Zoledronic Acid

Pharmacological Class and Group
Nitrogen bisphosphonates (Potent inhibitors of osteoclast-mediated bone resorption).
Mechanism of Action
Bisphosphonates are stable synthetic analogues of natural inorganic pyrophosphate that exhibit a selective and extraordinary chemical binding affinity for calcium hydroxyapatite crystals that cover the active mineralized bone matrix.
Osteoclastic Molecular and Cellular Mechanism
1. Selective Fixation to the Bone Matrix: After administration, the circulating bisphosphonate quickly binds to the hydroxyapatite surface in areas of active bone turnover.
2. Endocytosis by the Osteoclast: During the process of acid bone resorption, the osteoclast endocytoses the bisphosphonate released from the matrix into its cellular cytoplasm.
3. Inhibition of Farnesyl Pyrophosphate Synthase (FPPS): Nitrogen bisphosphonates (alendronate, zoledronate) potently inhibit the key enzyme FPPS in the mevalonate metabolic pathway:
Inhibition of FPPS Blockage of the synthesis of isoprenoid lipids (FPP and GGPP)
4. Lack of Protein Prenylation: When these lipids are depleted, the prenylation of small GTP-binding proteins essential for cell signaling (such as Ras, Rho, Rac and Cdc42) stops.
5. Loss of the Brush Edge and Apoptosis: The osteoclast loses its functional cytoskeleton and the ability to form the acidic brush border to dissolve bone, ultimately inducing apoptosis or programmed cell death of the osteoclast. This slows bone turnover and allows osteoblasts to progressively fill the cavities with dense mineralized bone matrix.

High Resolution Pharmacokinetics
- Absorption and Bioavailability: Extremely low orally for alendronate (just 0.6% to 0.7% on a complete empty stomach, reducing to practically absolute zero if ingested together with food, coffee, juices or divalent cations such as calcium). Zoledronic acid is administered exclusively intravenously, guaranteeing 100% systemic bioavailability.
- Distribution: Rapid uptake of 50% of the absorbed dose by bone tissue; the rest is distributed transiently in soft tissues before renal excretion. Low binding to plasma proteins.
- Metabolism: They are not metabolized in the liver in any way. They do not interact with CYP450 cytochromes.
- Excretion: Rapid excretion of circulating excess not fixed to the bone by renal glomerular filtration.
- Half-life (t1/2): The free plasma half-life is short (approximately 1.5 to 3 hours), but the final terminal bone elimination half-life is extraordinarily long, estimated at more than 10 to 12 years due to its irreversible retention in the depth of the inactive mineralized bone matrix.

Clinical Indications and Off-Label Uses
Indications of Alendronate
- First choice treatment in postmenopausal and senile osteoporosis in men for the prevention of vertebral, femoral and intertrochanteric fractures.
- Osteoporosis chronically induced by systemic glucocorticoids.
- Active Paget's Bone Disease.
Indications of Zoledronic Acid
- Senile osteoporosis (single annual dose of IV infusion).
- Malignant hypercalcemia: Rapid correction of elevated serum calcium secondary to active tumor bone lysis (Zometa).
- Prevention of major skeletal events (pathological fractures, spinal cord compression) in patients with bone metastases from solid tumors or multiple myeloma.
Dosage and Clinical Adjustment
Oral Alendronate (Osteoporosis):
- Standard recommended dose of 70 mg once a week orally (or 10 mg once a day, but with a lower adherence profile).
- Mandatory instructions for taking: Take the tablet immediately upon waking up in the morning, on an empty stomach, only accompanied by a glass of plain water (still, non-mineralized, minimum of 200 mL). The tablet should not be chewed or crushed. The patient should remain completely upright standing or sitting without lying down for the next 30 continuous minutes and until after having ingested the first solid food or drink of the morning, to avoid severe destructive esophageal reflux.
Zoledronic Acid EV (Osteoporosis):
- Dose of 5 mg administered as a slow intravenous infusion (infusion time not less than 15-20 continuous minutes) once a year. Ensure adequate pre- and post-infusion fluid intake.
Adjustment in Renal Failure: Both nitrogenous bisphosphonates are formally contraindicated if the stimulated glomerular filtration rate (eGFR) is < 35 mL/min/1.73 m² due to the lack of long-term safety studies and high cumulative risk.

Absolute and Relative Contraindications
Contraindications of Alendronate
- Esophageal abnormalities or delayed emptying: Esophageal stricture, achalasia, severe diffuse esophageal spasm.
- Physical inability of the patient to remain upright standing or sitting for a minimum of 30 continuous minutes after taking.
- Known hypersensitivity.
- Hypocalcemia established prior to onset.
- Moderate-severe kidney failure: eGFR < 35 mL/min/1.73 m² (risk of excessive bone accumulation).
Contraindications of Zoledronic Acid
- Hypersensitivity to zoledronate or related bisphosphonates.
- Severe symptomatic hypocalcemia of any etiology.
- eGFR < 35 mL/min/1.73 m² (high risk of precipitation-induced direct renal tubular toxicity).
Adverse Effects (ADR) and Specific Toxicity
- Alendronate (Common 1%-10%): Acute erosive esophagitis, severe gastroesophageal reflux, destructive esophageal ulcers or upper digestive bleeding, dyspepsia, arthralgias and initial non-specific bone myalgias of a transient nature.
- Zoledronic Acid (Frequent 1%-10%): Flu-like acute phase reaction after transient infusion in the first 48 hours (presenting with self-limited fever, diffuse myalgias, deep arthralgias, chills), secondary acute renal failure due to tubular toxicity (mitigated by slow infusion and adequate hydration of the patient).
- RAM Critical Class of Bisphosphonates (Rare <0.1%): Osteonecrosis of the jaw (ONJ) and atypical femoral fractures (AFF).
Critical Toxicity: Osteonecrosis of the Jaw (ONJ) and Atypical Fractures
1. Osteonecrosis of the Mandible (ONJ): It is characterized by the exposure of devitalized and necrotic maxillary or mandibular bone in the oral cavity, refractory to healing for more than 8 weeks. This complication is classically associated with the use of high oncological doses of EV bisphosphonates. It is favored by invasive dental procedures, tooth extractions or previous active periodontal disease. It is mandatory to perform a complete dental check-up and clean the mouth before starting bisphosphonates.
2. Atypical Femoral Fractures (AFF): Low-energy subtrochanteric or diaphyseal fractures of the femur (without impact trauma), usually preceded by deep dull pain in the thigh for months before the event. They are associated with prolonged and severe suppression of normal bone turnover (>5 continuous years), which prevents the natural physiological repair of common microfractures due to fatigue of the cortical femur. It is suggested to consider a "drug holiday" after 3-5 years of continuous use if the patient's fracture risk has stabilized at low clinical risk levels.
Drug Interactions of Clinical Relevance
- Multivalent cations (Calcium, Iron, Magnesium Supplements, antacids, milk): They completely inactivate the absorption of alendronate in the gastric lumen through the formation of insoluble chelation complexes. You should wait at least 30 to 60 minutes after taking alendronate before taking any food, drink or other oral drug.
- Nephrotoxic drugs (potent NSAIDs, aminoglycosides): Substantially increase the risk of acute renal failure due to tubular necrosis when coadministered closely with the infusion of zoledronic acid.
Safety in Pregnancy and Breastfeeding
- Pregnancy: Absolutely contraindicated. Bisphosphonates are deposited in maternal bone and released slowly over years, posing a theoretically severe risk of fetal harm (embryotoxicity and disturbances of skeletal calcium homeostasis in neonatal development).
- Breastfeeding: Not recommended. Robust clinical safety data are lacking to justify its use during this period.
