Mechanism
Mechanism of Action
Clopidogrel is a second-generation thienopyridine that acts by irreversibly modifying key platelet receptors for primary hemostasis.
Mechanism of Metabolic Activation and Inhibition of P2Y12
1. Clopidogrel in vivo is an inactive prodrug that requires two consecutive hepatic oxidative steps to transform into its active thiol metabolite.
2. The active metabolite circulates and is covalently linked through a disulfide bridge to the cysteine residue of the P2Y12 receptor on platelets.
3. The P2Y12 receptor is a G protein-coupled receptor (Gi) that responds physiologically to exogenous ADP released by activated platelets, suppressing intracellular adenylate cyclase and reducing cAMP levels.
4. By irreversibly blocking the P2Y12 receptor, clopidogrel prevents the downstream signaling cascade that activates the GPIIb/IIIa integrin. Since this glycoprotein complex cannot be activated, plasma fibrinogen is unable to cross-bind to neighboring platelets, irreversibly inhibiting platelet aggregation for the rest of the cell's half-life.
Pharmacokinetics
Pharmacokinetics and Liver Activation
- Rout of administration: Oral. Rapid but limited absorption (~50% actual absorption).
- Metabolism: Highly complex pharmacokinetics. 85% of absorbed clopidogrel is rapidly hydrolyzed by plasma esterases to an inactive carboxylic acid-derived metabolite. Only the remaining 15% are available to undergo sequential two-step hepatic activation in the microsomal P450 system:
- Step 1: CYP2C19, CYP1A2 and CYP2B6 form 2-oxo-clopidogrel.
- Step 2: CYP2C19, CYP2C9, CYP3A4 and CYP2A6 open the thienopyridine ring generating the active thiolic metabolite. - Excretion: Equal excretion of metabolites in urine (~50%) and feces (~46%) during 120 hours post-dose.
- Half-life (t1/2): Unaltered plasma clopidogrel has a half-life of 6 hours; The circulating active metabolite has an elimination half-life of 30 minutes after covalent binding to the exposed platelet.
Indicators and dose
Indications
- Prevention of atherothrombotic events in patients with a history of acute myocardial infarction, recent ischemic stroke, or established peripheral arterial disease.
- Treatment of acute coronary syndromes in combination with ASA (dual antiplatelet therapy - DAPT) in patients undergoing or not undergoing PCI.
- Prevention of thrombosis of vascular endoprostheses (coronary stent) after the intervention.
Dosage and Resistance
- Loading dose (SCA/ICP): 300 mg to 600 mg orally to achieve rapid platelet saturation in less than 2 to 4 hours.
- Maintenance dose: 75 mg orally once daily.
- Clopidogrel resistance (Slow Metabolizers): Subjects carrying loss-of-function alleles in the CYP2C19 gene (alleles *2 and *3) metabolize clopidogrel inefficiently, presenting low plasma concentrations of the active thiol metabolite and alarming ischemic recurrence rates. In these patients, the use of alternatives such as Prasugrel or Ticagrelor is preferred.
Pregnancy and Breastfeeding
FDA Category B. Clinical data in pregnant women is extremely scarce. It is only recommended to use if strictly necessary and under risk monitoring. It is unknown if it is excreted in breast milk.
Security
Contraindications
Absolute
- Hypersensitivity to clopidogrel or thienopyridine derivatives.
- Active pathological bleeding (eg, peptic ulcer or active intracranial hemorrhage).
- Severe liver failure with associated alteration of the synthesis of the hemostatic profile.
Relative
- Patients scheduled for urgent coronary bypass surgery (revascularization) within less than 5 days (high risk of massive perioperative bleeding).
- Chronically decreased liver or kidney function.
Adverse Effects (ADR)
- Hemorrhage: Relevant clinical incidence of purpuric hematoma, bleeding at puncture sites and respiratory or digestive tract bleeding.
- Severe hematological (Rare): Antibody-mediated Thrombotic Thrombocytopenic Purpura (TTP) (with microangiopathic hemolytic anemia, severe thrombocytopenia and impaired neurological and renal function). Neutropenia and idiopathic agranulocytosis less than 0.1%.
- Digestive disorders: Diarrhea, dyspepsia, mild epigastric pain.
Interactions
Critical Interaction with Proton Pump Inhibitors (PPIs)
Proton pump inhibitors (especially Omeprazole and Esomeprazole) are moderate to potent inhibitors of the enzyme CYP2C19. Co-administration of omeprazole with clopidogrel drastically decreases the conversion of clopidogrel into its active metabolite, substantially reducing its antiplatelet effect and increasing the risk of in-stent thrombosis and recurrent infarction. If a patient on DAPT with clopidogrel requires gastric gastric protection, Pantoprazole or Rabeprazole, drugs with a lower inhibition profile on CYP2C19, should be prescribed.
