Valproic Acid (Sodium Valproate)

A GABAergic tone modulator and ion channel blocker with a powerful mood stabilizing action and preventer of seizures.

Mechanism

Chemical and Commercial Profile

Common trade names: Depakine, Depakote, Valcote.
Group: Broad-spectrum antiepileptic, branched-chain carboxylic fatty acid.

Mechanism of Action

It shows a highly effective pleiotropic mechanism on neuronal excitability:

  1. Increased central levels of GABA (Gamma-Aminobutyric Acid): Irreversibly inhibits the enzymes responsible for GABA catabolism, such as GABA transaminase (GABA-T) and succinic semialdehyde dehydrogenase, simultaneously promoting the synthesis of GABA by glutamic acid decarboxylase (GAD).
  2. Voltage-gated Sodium channel blockade (Nav): Stabilizes overactive axonal membranes by prolonging the inactive state of sodium channels during repetitive high-frequency action potentials.
  3. Inhibition of low-threshold T-type Calcium channels: Key mechanism for the control of generalized absence seizures.

Pharmacokinetics

Pharmacokinetics

  • Absorption: Excellent and rapid orally; 100% bioavailability. Extended release formulations decrease peak concentration fluctuations.
  • Distribution: High binding to plasma proteins (90%), saturable at high concentrations.
  • Metabolism: Hepatic through conjugation with glucuronic acid (70%) and mitochondrial β-oxidation (20%). Weak involvement of the CYP450 system.
  • Excretion: Renal (95%) mainly in the form of conjugated metabolites.
  • Half-life (t1/2): 10 to 16 hours.

Indicators and dose

Indications

  • Treatment of acute mania and hypomania phases in bipolar disorder, and prophylactic maintenance (especially effective in patients with rapid cycling or mixed dysphoric mania).
  • Generalized and partial seizures, Petit Mal absence seizures, severe myoclonus.
  • Prophylaxis of refractory migraine in adults.

Dosage and Settings

  • Acute Bipolar mania: Loading dose of 20 mg/kg/day orally; Usual therapeutic range: 1000 mg to 2000 mg/day.
  • Target Valproic acid level (Monitoring): Maintain trough serum levels between 50 mcg/ml and 125 mcg/ml.
  • Kidney Failure: Adjust dose due to higher fraction of free plasma valproate due to protein displacement caused by uremia. Start with lower doses and monitor free valproate (not total).
  • Liver Failure: Absolute contraindication in moderate to severe liver disease.

Contraindications

Severe Teratogenicity Alert

Valproic acid is the psychotropic drug with the greatest known teratogenic potential. Its use in the first trimester is associated with an incidence of up to 10% of major congenital malformations, highlighting neural tube defects (myelomeningocele) due to direct interference with the cellular metabolism of folic acid. Contraindicated in women of childbearing age unless the strict measures of the government Pregnancy Prevention Plan are met.

  • Acute or chronic decompensated active liver disease.
  • Family history of severe liver dysfunction due to medication.
  • Known disorders of the mitochondrial respiratory chain (POLG gene mutations, Alpers-Huttenlocher syndrome).
  • Metabolic disorders of the urea cycle (risk of fatal hyperammonemic encephalopathy).

Adverse Effects (ADR)

  • Metabolic and Immunological: Rapid weight gain due to increased appetite, transient diffuse alopecia (residual wavy hair), benign distal postural tremor.
  • Hepatic and Digestive: Transient elevation of transaminases (AST/ALT) in 15% of patients, severe initial nausea, acid dyspepsia.
  • Hematological: Transient dose-dependent thrombocytopenia due to inhibition of megakaryocyte maturation, dysfunction of platelet aggregation.
  • Rare but very serious:
    • Fulminant hepatic failure: Jaundice, lethargy and early hepatic synthesis failure in children.
    • Acute hemorrhagic pancreatitis: Acute abdominal pain and vomiting; requires immediate amylase and lipase determination.
    • Hyperammonemic encephalopathy: Profound mental confusion and lethargy with normal transaminase levels, caused by ammonium accumulation due to failure of the urea cycle induced by valproate metabolites.

Clinical Interactions

  • Enzyme Inhibitory Effect: Selectively inhibits the isoenzyme CYP2C9 and UDP-glucuronosyltransferase (UGT). It dangerously multiplies the plasma levels of lamotrigine (extreme risk of Stevens-Johnson syndrome if not titrated slowly) and phenobarbital.
  • Carbapenems: Beta-lactam antibiotics of the carbapenem group (meropenem, imipenem) drastically plummet valproate concentrations in a matter of hours by inhibiting its absorption and enterohepatic recycling, precipitating severe epileptic seizures. Avoid coadministration.

Pregnancy and Breastfeeding

FDA Category: X (in migraine prophylaxis) or D (in epilepsy/bipolar disorder). Highly teratogenic (spina bifida, cleft palate, postnatal cognitive delay). Breastfeeding: Compatible with caution. Its passage into breast milk is extremely low due to its high binding to maternal plasma proteins; monitor the appearance of petechiae or hematomas in the newborn.
