Mechanism of Action
Estos polímeros de alta afinidad hidratante estabilizan las capas de la película lagrimal y protegen el epitelio corneal basal:
- Viscoelasticidad y Retención del Ácido Hialurónico: El hialuronato de sodio es un polímero glucosaminoglicano de elevado peso molecular. Presenta un comportamiento no newtoniano o pseudoplástico:
- Durante el Parpadeo (Alta Tasa de Cizallamiento): Su viscosidad disminuye de forma drástica bajo la fuerza de fricción del párpado, distribuyéndose uniformemente sobre la córnea y conjuntiva de forma cómoda, evitando la visión borrosa.
- Entre Parpadeos (Bajo Cizallamiento): Su estructura helicoidal macromolecular se expande, reteniendo moléculas de agua hasta 1000 veces su peso, estabilizando la película de lágrima y prolongando el tiempo de rotura lagrimal (TBUT).
- Bioadhesión de la Carboximetilcelulosa (CMC): Polímero aniónico de celulosa que interactúa con las glucoproteínas mucínicas del epitelio corneal basal. Al adherirse físicamente, protege la superficie ocular del roce palpebral y facilita la cicatrización de microerosiones estromales.
- Acción Osmoprotectora y Bioprotectora de la Trehalosa: Disacárido natural que penetra directamente en las células epiteliales deshidratadas de la córnea sometidas al estrés del ojo seco hiperosmolar:
- Prevención de la Desnaturalización Proteica: Forma un gel viscoelástico estabilizador transitorio alrededor de las proteínas celulares y lípidos de membrana, impidiendo su agregación irreversible y desnaturalización estructural inducida por la hiperosmolaridad.
- Preservación de la Viabilidad Celular: Reduce el daño por estrés oxidativo y disminuye la apoptosis celular conjuntival, protegiendo la superficie ocular del ciclo inflamatorio.

Security
Contraindications
- Absolute: Documented hypersensitivity to any of the components of the tear substitute.
- Relative: Use of formulas with quaternary ammonium preservatives (benzalkonium chloride) in patients with severe dry eye or who require artificial tears to be instilled more than 4 to 6 times a day (the accumulated preservatives exacerbate corneal epitheliotoxicity, inducing iatrogenic punctate keratitis; in these cases the exclusive use of single-dose formulations free of preservatives).
Adverse Effects (ADR)
- Ocular Sites: Mild and transient blurred vision after immediate dural dosage (associated with the high viscosity of hyaluronate or CMC, it subsides after a few blinks); transient ocular itching; accumulation of crystalline or sticky debris on the lower eyelid edge in the morning.
- Systemic: Non-existent with an absolute clinical course.
