Netarsudil
Netarsudil is the first representative of an innovative therapeutic class that increases conventional trabecular outflow, acting directly on the physiological site of greatest resistance in glaucoma.

Mechanism of Action
Netarsudil acts by blocking intracellular signaling mediated by the small GTPase Rho and its effector kinase (ROCK):
- Selective inhibition of ROCK-1 and ROCK-2: Reversibly blocks the kinase catalytic site of conventional trabecular meshwork cells and Schlemm's canal.
- Actomyocin Cytoskeleton Disorganization: By inhibiting the phosphorylation of myosin light chain phosphatase (MLCP), intrinsic cellular contractility and the density of actin stress fibers in trabecular tissue decrease. The mesh cells relax and contract, physically opening the cribriform filtration spaces.
- Additional Integrated Hypotensive Mechanisms:
- Reduction of Episcleral Venous Pressure: By relaxing the cell wall of the episcleral collecting vessels, the remaining IOP is directly reduced.
- Norepinephrine Transporter (NET) Inhibition: Increases local dural availability of norepinephrine, compensatory stimulation of additional adrenergic receptors and further reducing aqueous humor synthesis.

Netarsudil is a high local affinity molecule formulated in a water-soluble ophthalmic solution:

Security
Contraindications
- Absolute: Known hypersensitivity to netarsudil or any of the excipients of the eye drops.
- Relative: Pre-existing Fuchs corneal dystrophy (associated with worsening transparency or bullous keratopathy); severe hepatic or renal failure (although its systemic absorption is negligible, it lacks robust safety studies).
Adverse Effects (ADR)
- Local Ocular: Severe conjunctival hyperemia (occurs in up to 50% of treated patients, of direct physiological origin due to local vasodilation of the conjunctival and episcleral vessels due to ROCK inhibition); Cornea Verticillata / Vortex Keratopathy (accumulation of bilateral symmetrical lipid microcrystalline deposits in the cytoplasm of the basal corneal epithelium, with an asymptomatic course and completely reversible after stopping treatment); self-limiting punctate subconjunctival hemorrhages in the inferior fornix.
- Systemic: Little or no effects on heart rate, blood pressure or lung dynamics due to local ocular metabolism and no free systemic absorption.
