Arthrocentesis with counting, Gram, culture and crystals; acute phase reactants.

red flags

Very painful acute monoarthritis with fever → septic arthritis until proven otherwise.

Standard management

  • Do not infiltrate without arthrocentesis — antibiotic if septic; NSAID/colchicine yes microcrystalline
