Initial workup

Exploration (Lasègue, strength, reflexes); MRI only if alarming or refractory.

red flags

Saddle anesthesia, urinary retention or progressive motor deficit → cauda equina.

Standard management

  • Analgesia (NSAID/paracetamol) — early mobilization
  • Neuromodulator — yes neuropathic pain
