Initial workup
Reagent strip and urinary sediment; Urine culture if complicated or recurrent.
red flags
Dysuria + fever and low back pain (pyelonephritis), pregnant, male or sepsis → directed treatment and study.
Standard management
- Fosfomycin — uncomplicated cystitis
- Nitrofurantoin — cystitis (avoid in advanced CKD)
- Analgesia — symptomatic relief
