Neonatal hypotonia

Why it occurs

  • Hypoxic-ischemic encephalopathy or central brain injury (most common central cause due to perinatal compromise)
  • Spinal muscular atrophy (SMA type I or Werdnig-Hoffmann disease, lower motor neuron alteration due to SMN1 gene mutation)
  • Down syndrome or other chromosomopathies (frequent central genetic cause associated with dysmorphic traits)
  • Prader-Willi syndrome (deletion in paternal chromosome 15, characterized by initial severe hypotonia and feeding difficulties)
  • Transient neonatal myasthenia gravis (transplacental transfer of antibodies against the acetylcholine receptor)
  • Inborn errors of metabolism or congenital myopathies (mitochondrial disorders, glycogenosis type II or Pompe disease)

Initial workup

General analysis that includes serum creatine kinase (CK) (elevated in myopathies), blood gases to assess lactic acidosis, ammonium and amino acid/acylcarnitine profile (ruling out metabolic errors). Transfontanelar ultrasound and brain MRI (to identify structural lesions, neuronal migration or demyelination). Electromyogram (EMG) and nerve conduction velocity to differentiate second motor neuron, motor or muscle plate pathology. Specific genetic study (karyotype, comparative genomic hybridization using arrays, panel for SMA or methylation for Prader-Willi). Muscle biopsy in selected cases without genetic diagnosis.

red flags

Hypotonia associated with severe respiratory failure (diaphragmatic breathing, subcostal indrawing or apnea); severe difficulty swallowing with inability to suck or manage secretions (high risk of aspiration); total absence of archaic reflexes (Moro reflex, absent sucking and grasping); lingual fasciculations (pathognomonic of lower motor neuron involvement); cardiac arrhythmias or associated hypertrophic cardiomyopathy; deep lethargy with lack of response to painful stimuli.

Standard management

  • There is no generic drug for hypotonia; Treatment is directed at the underlying cause. Nusinersen or Onasemnogene abeparvovec — specific advanced gene therapies for spinal muscular atrophy, administered in specialized centers under strict protocols
  • Neostigmine or Pyridostigmine — acetylcholinesterase inhibitors indicated in confirmed cases of neonatal myasthenia gravis or congenital myasthenic syndromes; pyridostigmine dosage of 0.5 to 1 mg/kg orally every 4-6 hours
  • Carnitine, coenzyme Q10 or group B vitamins (mitochondrial cocktail used empirically or confirmed in disorders of cellular energy metabolism).
