Initial workup

ECG in all; history of the episode and orthostatism; echocardiogram/Holter if cardiac cause is suspected.

red flags

Exertional syncope, without prodrome, with palpitations or murmur → suspect cardiac cause (risk of sudden death).

Standard management

  • Treat the cause — avoid hypotensive drugs if orthostatic
