Why it occurs
- Constrictive pericarditis
- Advanced restrictive cardiomyopathy
- Acute right ventricular myocardial infarction
- Severe tricuspid regurgitation with right ventricular dysfunction
- Cardiac tamponade (occasionally positive, although less common than in constriction)
- Critical tricuspid stenosis
Initial workup
Transthoracic Doppler echocardiogram to evaluate diastolic filling hemodynamics, looking for respiratory variations in transvalvular flows, rectification of the interventricular septum during inspiration ("septal bounce"), pericardial thickening and dilation of the inferior vena cava without inspiratory collapsibility; Simultaneous left and right cardiac catheterization to record intracavitary pressures (search for the "square root" or diastolic dip-and-plateau pattern characteristic of pericardial constriction); Computed tomography or cardiac MRI to visualize the pericardium (thickness >4 mm confirms constrictive pericarditis).
red flags
Paradoxical increase (or absence of the normal decrease) in jugular venous pressure and distention of the neck veins during spontaneous inspiration, associated with systemic arterial hypotension, rapid and weak pulse, severe dyspnea at rest, orthostatic syncope or presyncope, deep peripheral pitting edema, or painful congestive hepatomegaly with tense ascites. It indicates a severe restriction to the diastolic filling of the right ventricle (which is unable to accommodate the increased venous return during inspiration), constituting a sign of severe hemodynamic compromise due to restrictive pericardial or myocardial pathology that requires urgent specialized evaluation.
Standard management
- Furosemide — loop diuretic indicated with caution to reduce general venous congestion, closely monitoring not to excessively reduce the preload on which these patients depend to maintain cardiac output; 20 to 40 mg orally or intravenously daily
- Spironolactone — aldosterone antagonist indicated if there is ascites or marked hepatic congestion; 25 to 50 mg per day orally
- 0.9% isotonic saline solution — indicated temporarily and with a slow drip in the acute phase of right ventricular infarction to optimize preload before revascularization; 250 mL controlled infusion
- Enoxaparin (indicated if anticoagulation is required due to associated atrial fibrillation; 1 mg/kg subcutaneously every 12 hours).
