Senile skin vulnerability syndrome

Why it occurs
- Massive decrease in the levels of collagen type I and III and elastic fibers in the dermis
- Atrophy of the dermoepidermal junction with flattening of the dermal papillae, facilitating shearing
- Chronic use of high-dose systemic or inhaled corticosteroids for respiratory or rheumatic pathologies
- Chronic skin dehydration due to decreased secretion of sebum and sweat
- Chronic venous insufficiency with stasis and hemosiderin deposition that weakens the subcutaneous cellular tissue
Initial workup
Visual clinical assessment using the International Skin Tear Advisory Panel (ISTAP) classification; determination of total proteins, albumin, prealbumin, serum levels of zinc and vitamin C; complete blood count and coagulation; soft tissue ultrasound if deep dissecting hematoma is suspected; microbiological culture by smear or tissue biopsy only if there are unequivocal signs of active bacterial infection.
red flags
Extensive skin tears (ISTAP type 3 classification, with total loss of the skin flap), signs of local necrotizing infection with rapidly progressive erythema, disproportionate pain and gas crepitation on palpation, deep dissecting hematoma in the lower extremities that compromises the viability of the limb due to compartment syndrome, or sepsis of cutaneous origin.
Standard management
- Acid-based moisturizing creams — pH 5.5) with 5-10% urea or hyaluronic acid (applied twice a day to the extremities gently, without excessive friction
- Vitamin C — 500 mg orally per day, adjuvant for collagen synthesis and stabilization of the dermoepidermal junction
- Soft silicone or hydrogel dressings to cover active skin tears (absolutely avoiding strong adhesives that drag the epidermis when removed).
