Initial workup
Urinary sediment, ultrasound or CT without contrast; kidney function.
red flags
Colic with fever and involvement → obstructive pyelonephritis (urgent drainage); in >60 years rule out aneurysm.
Standard management
- NSAID (diclofenac) — first line of colic; opioid pain reliever if it doesn't go away
