EpistemisFacial telangiectasias

Why it occurs
- Erythematotelangiectatic rosacea (dysfunction of innate immunity and neurovascular dysregulation of the face)
- Chronic exposure to cumulative solar radiation (actinic damage to the dermal vascular connective support)
- Damage induced by the inappropriate use of high-potency topical corticosteroids applied to the face for a long time
- Chronic alcohol consumption (persistent reflex vasodilation)
- Collagen diseases such as progressive systemic sclerosis (scleroderma, typically with tapetate or punctate morphology).
Initial workup
Detailed visual clinical evaluation and digital dermoscopy of the cutaneous vessels. If there is suspicion of systemic involvement or Rendu-Osler-Weber syndrome: upper gastrointestinal endoscopy, chest CT (search for pulmonary arteriovenous malformations), complete blood count to rule out chronic iron deficiency anemia due to occult bleeding.
red flags
Presence of multiple telangiectasias of systemic distribution affecting the oral mucosa, tongue, lips or nail beds, accompanied by recurrent episodes of epistaxis without apparent cause (suggestive of hereditary hemorrhagic telangiectasia or Rendu-Osler-Weber syndrome).
Standard management
- Brimonidine tartrate gel 0.33% — selective alpha-2 adrenergic receptor agonist that produces temporary vasoconstriction of the erythematous capillary bed, applied daily in the morning
- Oxymetazoline cream 1% — topical vasoconstrictor with similar mechanism of action for persistent erythema
- Strictly avoid physical triggers such as extreme heat, spicy foods and alcohol
- Specialist referral for physical destructive therapy with pulsed dye laser (LCP) or intense pulsed light (IPL) if anatomical resolution is required.
